First Encounter

The scene of Sebastian Castellanos' encounter with a Haunted Oscar Connelly is a direct reference to the reveal of the first zombie in Resident Evil, complete with a dropped and halfway-eaten head.

Dear Creator

During the sequence where the player must run from Ruvik, there is an area with a ladder going down. Descend this ladder and look to the left to spot a book with the logo of Tango Gameworks on the cover.

Music posters

Several posters of music bands can be found in Chapter 11. One of these is for a music group named "Box Punk" with the three band members wearing safes like that of The Keeper, referencing the electronic band Daft Punk. There is also a poster for a band named "Dead Lloyd", which might be a reference to Pink Floyd.

Self-reference film posters

Several film posters found in Chapter 11 bear references to certain characters in the game. The one for "Black Hair Curse" features the hair-concealed face of Laura and is done in the same style as The Grudge, complete with a variation where a wide-open eye is featured à la the poster of The Grudge 2. Others found around the map include "Serbian Psycho", "White Fog" and another called "Zom Bomb" is about a single man fighting against a horde of zombies carrying various tools and weapons.

Tango Denim

After getting out of the elevator at the beginning of Chapter, there is a poster to the left advertising "The perfect white shirt" by "Tango Denim", which is a reference to Tango Gameworks. This can also be seen briefly in Chapter 1 as a falling billboard.

ZeniMax crossovers

Several posters found in Chapter 13 are references to various games and series made by the many subsidiaries of ZeniMax Media. These being the "Enjoy Vault Living" and "Sugar Bombs" from the Fallout series, and ones of The Golden Cat and The Hound Pits Pub from Dishonored.

Juli Perfume

When locating the first train car in Chapter 14, a perfume ad poster can be found with a pre-release screenshot of Juli Kidman as its picture.

Babies Galore

Interacting with the vending machine in Chapter 14 will cause it to dispense a small mob of The Babies.

Creepy Sister/The Girl in the Red Dress

Before heading into Biometrics, there are two broken doors leading into a dark room that will continuously open and shut. If the player shine their flashlight past the doors into the room, they'll see a creepy image of Laura Victoriano in a red dress standing there while giggling eerily. Staring at her long enough will cause her to rapidly teleport towards the doors each time they shut until she's directly in front of the player and looking right at them.

Sword Oda and Bikini Kid

While fighting a Haunted Joseph Oda, there is a usable film projector that distracts him momentarily with image slides. The projector will briefly display two images, one of Joseph holding a katana and one of Juli wearing a bikini. The projector will shut off shortly afterwards.

Kidman's Desk

While navigating past the Krimson City police precint just prior to the flashback inside Sebastian's office, there s a desk in the larger room that's strangely devoid of content. Shine the flashlight at it to make Kidman's personal computer appear.

Private Dance

After crossing a makeshift bridge between two broken buildings into another office, there is an interactible vending machine with the BRAIN logo on its cover. Interact with this machine 15 times and back away, a short quake will rock the building and cause the painting on the left-side wall to fall down, revealing a peephole into a hidden room where Shade can be seen dancing to an audience of three Haunted with glowsticks. The dance stops after a few seconds and Shade will scream at the hole, seemingly aware of her being peeped on the whole time.

Stick to the Script

After arriving at the large STEM room where everyone is connected to the machine, shoot Ruvik´s brain container instead of following Leslie upstairs and the game will cut to black with the credits rolling. After a few seconds the game will cut back in with an incredulous Kidman exclaiming in confusion and unlocking the Stick to the Script achievement.

Group photo

Completing The Consequence shows a picture of Sebastian, Joseph and Juli together with the words "Congratulations! You did it!" written on the glass.

Crows

After defeating the Sadist at the end of Chapter 3, climb up the ladder leading up to a small room with a statue containing a key and look out the window to see a crow. Shoot this crow to trigger a short cutscene where five additional ones appear around the area. A hidden 2-minute timer will immediately begin counting down, and killing all five crows before it expires will spawn a pool of Green Gel 100,000 at the final gate.

Key is for the Past

After defeating Valerio Jimenez, check the note behind the corpse on the table and inspect it. A small pool of Green Gel will appear in place of the key if it was obtained previously.

  • Chapter: The Patient (New Game+ only, must have obtained the key during first playthrough)

Bonfire

Immediately after obtaining the previous secret, head outside to a large bonfire in the hospice yard where a Haunted can be seen carrying a woman towards it. Shoot and kill the Haunted before it is able to toss the woman into the fire and she will run towards the nearest shack before disappearing and leaving behind a pool of Green Gel 2,000.

Giant Pig

Burning all hanging bodies in the area spawns a giant pig in the yard close to the bonfire. Kill it to net Green Gel 1,000 from its corpse.

Brain Juice

The vending machines with the "Brain" logo found in the city dispense Green Gel vials when interacted with. Occasionally they will dispense motion sensor bombs that can be disarmed for trap parts and a goddess statue. It usually takes around three interactions to empty them out.
